Overview: SMART Goal Setting & Life Plans
The Welcome Home Database provides support for participant SMART Goal Setting and life plans. The Welcome Home Collaborative uses SMART Goal Setting as an informal evaluation to determine the needs and goals of it's participants.
A Life Plan is a way to take the results of a SMART Goal Setting evaluation and establish specific, measurable goals, strategies, and steps.
In the WHDB, SMART Goal Setting consist of one or more Domains (broad assessment categories). Each domain has one or more status associated with it.
Life Plans are not directly tied toSMART Goal Setting, but they can be configured mirror domains. Life Plans allow the user to establish specific goals for the participant, as well as defining specific stategies and steps for achieving these goals. Progress can be monitored for each strategy/step.
